The Transition Project is a group of school and agency staff and families that has collaborated together for over 40 years to ensure a smooth process of transition to adult life for our special education students.  There are a number of activities that are offered on a regular basis to maintain the ongoing relationships between school staff, agency staff and our students and their families as they prepare to leave public K-12 school and enter adult life.
